We don't really want people to access 'qsymm.kwant_continuum'

Bring format in line with keepachangelog.com

This is not actually required for using qsymm. Anyone installing
qsymm via pip is not going to have access to the notebooks anyway.
Anyone using qsymm by cloning the repository would be better served
by a conda 'environment.yml' file in the repository root (this could
also include e.g. plotting and Kwant).

This is deprecated in newer versions of numpy, and we do want to
avoid building up the entire list in memory.

99% of people will never need to make a release.

+ Introduce `BlochModel` as a subclass of `Model` in order to store
  Bloch Hamiltonians with floating point hopping vectors.
+ Change the way equality of `Model`s is tested, the current implementation
  treats tolerances more consistently, this fixes some bugs in symmetry finding.
+ Allow using floating point rotation matrices in `PointGroupElement`.
+ Add `bravais_point_group` to find the point group of a Bravais lattice using
  the translation vectors only. This is intended for generating candidades for `symmetries`.
+ Several bugfixes and internal code restructuring.

Many of these changes were made in anticipation of integrating `qsymm` with `kwant`,
allowing conversion between the Hamiltonian formats used.
